Our Core Approach
Fully interconnected platform
Healthful is a digital health platform to close gaps in care for people dealing with multiple chronic conditions.
A new software platform focused on addressing the complex interplay between cardiovascular, kidney, and metabolic diseases, particularly given the high prevalence of these conditions and the existing gaps in clinical care.
We want providers to focus on what the patient is experiencing, particularly their inter-related health problems, and to facilitate collaborative and coordinated approaches to care. Our interdisciplinary approach is supported by educational strategies that educate the populations at greatest risk.
In health centers or regions with a lower density of specialists, we have HIPPA-compliant video chat feature for consultation and follow-ups, to help overcome existing variability in access and prolonged wait times.
Cardiovascular Diseases
Cardiovascular disease is a group of diseases affecting your heart and blood vessels. It affects people of all ages, ethnicities and socioeconomic levels.
Almost half of all adults in the U.S. have at least one form of heart disease. Despite progress in treatment and management, CVD still has a large health, mortality, and economic burden.
We offer a more comprehensive approach, addressing the complex interplay between cardiovascular, kidney, and metabolic diseases.
Diabetes
In the US, roughly 38.4 million people have diabetes. Type 2 diabetes accounts for the majority of cases. Diabetes can lead to serious health complications, including heart disease, stroke, kidney failure.
We offer a more proactive approach, routinely assessing patients for multiple risk factors such as hypertension, diabetes, and chronic kidney disease, and implementing evidence-based guidelines for early intervention.
Arthritis
Arthritis affects health and functioning and is a leading cause of disability. About 53 million U.S. adults have arthritis.
Studies have shown a high prevalence of arthritis among individuals with other chronic diseases like heart disease and diabetes.
Thus, focusing on conditions alone is insufficient to address the excess disease burden caused by inter-related risk factors.
Chronic Kidney Disease
The interaction between kidney and cardiovascular diseases has long been recognized.
Only 40% of diabetes patients are screened for kidney diseases annually, we need to improve this to impact patient outcomes truly.
We offer a platform(tailored treatments) that consider the interdependencies of these conditions to improve patient outcomes.
